本人願意遵守 O2 Pilates 所有條例與規章,並克制行為以顧及自身及他人安全。
I am willing to abide by all regulations and rules of O2 Pilates and restrain my behavior to ensure my own safety and that of others.
本人明白及自願承擔因參加活動或課堂、使用場地、器材或設施而可能引致的損傷或疾病的風險。如本人因參加活動、使用場地、器材或設施而導致個人之身體或精神損傷、經濟損失或其他傷害,本人明確接受及同意 O2 Pilates 將毋須對此承擔任何責任。
I understand and voluntarily assume the risks of injury or illness that may result from participating in activities or classes, using venues, equipment, or facilities. If I suffer any physical or mental injury, economic loss, or other harm due to participating in activities, using venues, equipment, or facilities, I expressly accept and agree that O2 Pilates will not be liable for this.
本人同意免除O2 Pilates及其代理人、服務人員和僱員的責任,不會因本人參加活動或課堂、使用場地、器材或設施而可能蒙受的損傷、疾病、死亡、遺失或傷害向該機構和人士進行任何索償。
I agree to exempt O2 Pilates and its agents, staff, and employees from liability and will not make any claims against the institution and individuals for any injury, illness, death, loss, or harm that may result from participating in activities or classes, using venues, equipment, or facilities.
本人僅在此聲明本人身體狀況良好,亦無任何健康問題、功能受損或殘障情況顯示本人不宜使用O2 Pilates內之任何設施。
I hereby declare that I am in good physical condition and have no health problems, impairments, or disabilities that would make me unfit to use any facilities at O2 Pilates.
本人明白如對本身的健康或身體方面有任何懷疑,應在使用本中心設施/課堂前,徵詢醫生的意見。
I understand that if I have any doubts about my health or physical condition, I should consult a doctor before using the facilities/classes at this center.
取消課堂 Cancellation Policy
取消或更改課堂必須於開課前至少24小時前於 O2 Pilates 線上預約系統完成。如學生於課堂開始後10分鐘內未能出席課堂,或缺席而且無相關身體不適證明(由香港註冊醫生所簽發之上課當天或翌日的醫生紙),系統會視之為缺席並扣除通行證(醫生紙需於當月內補交,截止日期為當月最後一天,逾期將不予處理)。如果學生於課堂開始後10分鐘未能到達,將不能進入課室上課,系統亦會扣除通行證。
Class cancellations or changes must be completed at least 24 hours before the class starts via the O2 Pilates online booking system. If a student fails to attend the class within 10 minutes after it starts, or is absent without relevant medical proof (a medical certificate issued by a registered doctor in Hong Kong on the day of the class or the next day), the system will consider it as a no-show and deduct the pass (The medical proof must be submitted within the same month, with the deadline being the last day of the month. Late submissions will not be accepted.). If the student cannot arrive within 10 minutes after the class starts, they will not be allowed to enter the classroom, and the pass will also be deducted.
於緊急情況下,課堂導師或會更改而不作另行通知;取消政策仍然生效於此情況。
In emergency situations, the class instructor may change without further notice; the cancellation policy still applies in this situation.
課堂名額爆滿後,將不能報名。
If the class is full, no further registrations will be accepted.
組課堂最少3人成班,若未能成班,將於開始前12小時由 WhatsApp 通知。
Group classes require a minimum of 3 participants. If the class cannot be formed, a notification will be sent via WhatsApp 12 hours before the start.
購買課堂 Purchase Policy
套票有效期由第一堂開始計算,同學需於購買日計起一個月內上第一堂。成功購買後,請於限期內完成課堂。限期過後,剩餘之課堂將自動取消。所有套票延期申請需要有醫生紙(清楚顯示姓名、日期、診症原因、醫生簽名)或離港證明(清楚顯示姓名、去程及回程日期)才能處理。延期證明須於套票有效期內,每套票最多只能延期一次。於任何情況下,套票將不得轉讓或退款。
The validity period of the package starts from the first class attended, and students are required to attend the first class within one month of purchase. Upon successful purchase, please ensure completion of the classes within the specified deadline. Any remaining classes will be automatically forfeited after the expiration date. All requests for extensions of the package require a medical certificate (clearly displaying name, date, diagnosis, and doctor's signature) or proof of departure (clearly indicating name, departure, and return dates). The extension request must be submitted within the validity period of the package, and each package can only be extended once. The package is non-transferable and non-refundable under any circumstances.
條款及細則 Terms and Conditions
如香港天文台在營業時間前發出8號(或以上)熱帶氣旋信號或黑色暴雨信號,我們將暫停營業。受影響課堂將不會扣取任何通行證,同學可於app內重新預約課堂。一旦信號取消,我們將於2小時內恢復營業,同學須按時出席課堂。但若8號(或以上)熱帶氣旋信號或黑色暴雨信號在當天最後一堂課程開始前 3 小時內取消,我們將保持關閉狀態,當天所有課堂將取消,客人可重新預約課堂。
在3號熱帶氣旋信號、黃色暴雨信號或紅色暴雨信號下,我們將照常營業,同學須按時出席課堂。
If the Hong Kong Observatory issues a No. 8 Signal (or above) or a Black Rainstorm Warning, classes will be cancelled. Affected customers may reschedule their classes through the O2 app. Once the signal is lowered, we will resume within 2 hours and customers are required to attend the class as usual. However, if the No. 8 Signal (or above) or a Black Rainstorm Warning is cancelled less than 3 hours before the start of the last class of the day, we will remain closed and all classes for that day will be cancelled. Customers may reschedule their classes through the O2 app.
Classes will continue as scheduled under No. 3 Signal, Yellow and Red Rainstorm Warnings. Customers are required to attend the class on time.
O2 Pilates 不會對任何於設施中遺留的個人財產承擔任何責任。被遺留的物品將保存一星期。
O2 Pilates will not be responsible for any personal belongings left in the studio. Items left behind will be kept for one week.
迎新優惠只適用於全新顧客,每位顧客只可登記/使用一次,如有發現重複使用者,有權要求顧客補回套票差額。
The trial offer is only applicable to new customers, and each customer can register/use it only once. If duplicate use is found, we reserve the right to request the customer to reimburse the price difference for the package.
公司保留權利更改上述守則,如有任何爭議,公司保留解釋此條款及細則的最終決定權。
Our company reserves the right to modify the above regulations. In the event of any disputes, we retain the ultimate authority to interpret the terms and conditions.
退款條款 Refund Policy
付款後即已同意以下之取消政策:所有課堂如需改期必須24小時前自行於網上取消。所有已付款之產品均不可退款。
Upon payment, you are deemed to have accepted the following cancellation policy: Any rescheduling of classes must be done online at least 24 hours in advance. All paid products are non-refundable.
